Health Suite
All medications must be delivered to the Health Suite in their original containers. Please contact a nurse to schedule a drop-off before the first day of school. No medication will be administered without proper documentation on file.
Required for all New Students (PreK–8)
New Students-
Parent Health AssessmentCompleted by a parent or guardian (page 2 of the MDH form)

Physician Health AssessmentCompleted by your child's physician

OTC Medication Consent FormRequired for every student — signed by a physician. One form per student; families with multiple children may share one bag of stock medication.

Immunization RecordCurrent Maryland immunization certificate required

Lead Testing CertificateRequired if entering PreK, Kindergarten, or 1st Grade

Prescription Medication During the School Day
As Needed-
Medication Administration Authorization FormRequired for all prescription medications and any OTC medications not on the standard OTC form — must be signed by both the physician and parent/guardian

Asthma Action PlanRequired for students with asthma who may need medication during the school day

Allergy and Anaphylaxis Medication Administration Authorization PlanRequired for students with severe allergies who may need emergency medication
Over-the-Counter (OTC) Medications
The OTC form covers a list of optional, discretionary medications for occasional use to alleviate minor discomforts and help avoid early dismissals. The school nurse dispenses all OTC medications based on clinical assessment. You must supply the medication to the Health Suite. If you do not want your child to receive any OTC medications, you must still complete the form and indicate that.
Medication Drop-Off
All medications must be delivered to the Health Suite in their original containers. Please contact one of the school nurses to schedule a drop-off appointment and allow time for reviewing medication receipts and completed forms. Have all medications ready by the first day of school.
